Add and target forms in a report

Add one or more forms to a report from your library, or build a new one in place. Then, if not every organization should see every form, target specific forms to specific organizations.

Before you start:

  • Ensure your UpMetrics login is assigned an Org Admin or Editor role. Learn more about roles and permissions.

  • To add an existing form, have one or more forms in your form library. To build a new form with question groups, the question groups must already exist in your question library.

Good to know: By default, every form you add goes to all organizations in the report. Use targeting only when some organizations shouldn't see a form.

Caution: Changes you make to a form in a report also change that form in your form library. If you need a variation of a form for one report, make a copy in the form library first.

Add an existing form to a report

  1. In the left navigation pane, click Reporting > Report Builder. The Reports screen displays.
  2. If you're not already in the report you want to work with, click the Continue Editing button next to the reporting period you want to edit.
  3. Click the Add forms tab.
  4. Click the Edit form list button.
  5. Click the Browse form library button. The Add from Form Library window appears.
  6. Use the Search field to find a form, or scroll through the list, and select the checkbox next to each one you want.
  7. Click the Select forms button. The window closes and the selected forms appear in the Forms list, with their questions and any conditional logic shown alongside.
  8. Click the Save changes button above the Forms list.

Note: Each form can only be added to a reporting period once.

Build a new form directly in the report

  1. Click the Edit form list button above the Forms list, click the Add form link, and select Create new form. A Create new form window appears.
  2. Enter a name for the new form in the Form name field.
    Note: The new form is automatically assigned to the Program associated with the report, but you can add it to additional Programs.
  3. Select the checkbox next to any additional Programs you want to assign it to.
  4. Click the Create form button. The new form appears in the Forms list, empty.
  5. Click the Save changes button above the Forms list.
  6. Click the Edit button above the Form Questions area, then add questions, question groups, and conditional logic as needed.
  7. Click the Save question changes button above the Form Question list.

Note: Changes to questions within a report aren't applied until you publish it. New forms are also saved to your form library.

Target a form to specific organizations

  1. Click the Edit form list button above the Forms list.
  2. Select a form from the Forms list, then click the Assign organizations button. An Assign organizations window appears.
  3. Click the Specific organizations tab.
  4. Use the Search field to find an organization, or scroll through the list, and select the checkbox next to each one that should be assigned the form.
  5. Click the Assign forms button. The window closes and the form displays a Targeted tag along with how many organizations are assigned.
  6. Click the Save changes button above the Forms list.

Tip: On the Review & Publish tab, you can preview the report as a specific organization to confirm they see only the forms meant for them.

Reorder or remove forms

  1. Click Edit form list above the forms list.
  2. To reorder, drag the forms into the order you want. Responding organizations see forms in this order.
  3. To remove a form, click the trash can icon next to it. This removes the form from this reporting period only. It isn't deleted from your form library.
  4. Click Save changes.
