Create a form and add questions
A form is a set of questions and question groups that responding organizations complete as part of a report. Create a new form, add content from your library, then publish it.
Before you start:

Ensure your UpMetrics login is assigned an Org Admin or Editor role. Learn more about roles and permissions.
- To add existing content, have questions or question groups in your question library. You can also create new questions directly in a form, but question groups can only be created in the question library.
Create a form
- In the left navigation, click Reporting > Form Library. The form library appears.
- In the upper right corner, click Create new form. A dialog appears.
- Enter a name for the form in the Form name field.
- Optional: select the checkbox next to one or more programs the form belongs to.
- Click Create form. The form opens.
Add questions to a form
- Click Edit. The form becomes active for editing.
- Add content using any of these options:
- Existing questions: Click Add and select Question from library. Search or scroll to find questions, select the checkbox next to each one you want, and click Add x questions.
- New questions: Click Add and select New question, then choose a question type and complete its fields. New questions are saved to your question library automatically.
- Question groups: Click Add and select Question group. Search or scroll to find groups, select the checkbox next to each one you want, and click Add x groups.
- At the top of the questions list, click Save question changes.
Note: Changes to questions within a form aren't applied until you publish them. Unpublished questions show an (unpublished changes) tag.
Preview and publish the form
- Click Preview form to see the form as responding organizations will. Click Close preview when you're done.
- In the upper right corner, click Publish. A dialog shows how the change will affect other parts of reporting. Click Publish form. A success notification appears and your changes are published.
Edit a form
- In the left navigation, click Reporting > Form Library.
- Click the name of the form you want to modify, then click Edit.
- Make your changes:
- To edit a question, click its drop-down arrow to view its properties, then modify its fields as needed.
- To set whether a question is required, select or clear the Required checkbox.
- To reorder a question or question group, click and drag its handle icon.
- To duplicate or remove a question, click the icon next to it and select Duplicate or Remove. Removing a question from a form doesn't delete it from your question library.
- At the top of the questions list, click Save question changes.
- Click Preview form to check your changes. Click Close preview when you're done.
- In the upper right corner, click Publish. Select the checkbox to confirm your changes, then click Publish form.
Note: Question groups can only be modified in the question library, not from within a form.
Delete a form
- In the form library, click the icon next to the form and select Delete.
- Select the checkbox to confirm, then click Delete. The form is deleted.
Warning: Deleting a form is permanent. Confirm it isn't in active use on a report before continuing.
